I, FT Simmons a native of the state of Tenn and now a citizen of Tennessee, resident at Lennox in the county of Dyer in said State of Tennessee, who was a soldier from the State of Tenn, in the war between the United States and the Confederate States, do hereby apply for aid under the Act of the General Assembly of Tennessee, entitled "An Act for the benefit of the indigent and disabled soldiers of the late war between the States, and to fix the fees of attorneys or agents for procuring such pensions, and fixing a penalty for the violations of the same." And I do solemnly swear that I was a member of________.

In the service of the Confederate or United States, and that by reason of disability and indigence I am now entitled to receive the benefits of the Act. I further swear that I do not hold any National, State, or County office, nor do I receive aid or pension from any other State, or from the United Stated, and that I am not inmate of any Soldier's home, and that I am unable to earn a reasonable support for myself and family. I do further solemnly swear that the answers given to the following questions are true:

                   In what County, State, and year were you born?
                    Answer – Born in Barron Co Ky Feb 11th 1839

                    When did you enlist and in what command? Give the names of the regimental
                    and company officers under whom you served.
                    Answer – In Dyersburg April 1861 Capt O F Straul [Strahl] reg 4 Tenn
                    Cono [Colonel] Neely

                    In what battle or battles were you engaged, and, if not wounded, state what
                    disabilities did you receive, if any?
                    Answer – Bat Belmont Shilo Perryville Murfreesboro was the Batt. I was
                    wounded. I was shot in the left leg and Broken and have been partly paralised
                    ever since.

                    What was the precise nature of your wound or disability, if any?
                    Answer – I was wounded and sent to the hospital and stayed there 13 months and
                    am a cripple for life.

                    Were you incapacitated for service by reason of said wound or disability?
                    Answer – I was

                    Were you discharged from the army by reason of said wound or disability?
                    Answer – I was retired by the medical board of Cheatham Division

                    If discharged from the army, where were you and what did you do until the close
                    of the war?
                    Answer – I carried the mail through the enemy lines to west Tenn & return

                    What was the name of the surgeon who attended you?
                    Answer – Dr. Cannon who had charge of the hospittle at Chattenooga. Name of
                    the Hospittle was Gilmer Hospittle at Chattanooga Tenn.

                    How did you get out of the army, when and where?
                    Answer – I was with the army all the only while carrying mail Til the war closed
                    and surrendered at Dyersburg Tenn.

                    Did you take the oath of alegience to the United States Government?
                    Answer – I did

                    If so, when and under what circumstances?
                    Answer – At Dyersburg as the armies had all surrendered

                    Are you married, or have you been married?
                    Answer – Yes

                    If so, what is the size of your family living together?
                    Answer – Two children. Wife is dead

                    What are the respective ages of your wife and the children living with you?
                    Answer – none live with me 31 – 26 years

                    To what sex do your children belong?
                    Answer – Boy and Girl

                    Are not some of your children able to support you?
                    Answer – no, they have all they can attend to at home

                    In what business are you now engaged, if any, and what do you earn?
                    Answer – Running a little ferry on Obien river. It don't make me a good living.

                    What estate have you in your own right, real and personal, and what is its value?
                    Answer – I have none at all.

                    What estate has your wife in her own right, real and personal, and what is its
                    value?
                    Answer – She had none.

                    How have you derived support for yourself and your family for the last five
                    years?
                    Answer – I have no family.

                    Do you use intoxicants to any entent?
                    Answer – No, none at all.

                    How long have you been an actual resident of the state of Tennessee?
                    Answer – ever since 1858

                    Have you an attorney to look after this application?
                    Answer – no

                    Is so give his name and address.
                    Answer – none

State of Tennessee, Dyer County, Personally appeared before me, Tom Ferguson [UNREADABLE] of said County, the above named F. T. Simmons the applicant, with whom I am personally acquainted, and having the application read and fully explained to him, as well as the statements and answers therein made, made oath that the said statements and answers are true.
                    Witness my hand and seal of office, this 3" day of Aug 1903.

                    He has a wound in left leg below knee caused from a bullet passing through leg. The leg is somewhat perished and there is a partial paralysis of left side of entire body. His age is one disability.

State of Tennessee, Dyer County, Personally appeared before me, Tom Ferguson [UNREADABLE] of said county, the above named T. F. Ledsinger and T. M. Hambrick, two of the subscribing witnesses to the foregoing application, with whom I am personally acquainted, and known to me to be citixens of veracity and standing in this community, and who make oath that they are personally acquainted with the foregoing applicant, and that the facts set forth and statements made in this application are correct and true, to the best of their knowledge and belief, and that they have no interest in this claim, and that said applicant's habits are good and free from dishonor. And they further make oath to the following facts touching the applicant's service in the Confederate army: They know that he was in said army and made a good & faithful soldier and served through the [UNREADABLE].

Dear find inclosed Blank filled out for pansion that ought to have been in full force for the past ten years But my old friend Simmons has tride to live without any aid from the State But he has failed and can't do so. He is more than worthy of all he may get. I want you to start him from this quarter as he is in great need of any thing he can get. My friend Theo F. King is well acquainted with th old veteran F. T. Simmons and I before you will attend to his case as soon as you get this application as I no he is worthy from start to finish and a better soldier never was in the confederacy than Floury Simmons. This is from you friend J. G. Meadows Co. K. Second Tenn Regmnt Calvery Buford Division Bill Bigade and NB Forrest Command.
