No. 121                        CERTIFICATE OF PERMANENT LICENSE.                          SERIES B

We, The State Board of Medical Examiners of Tennessee, appointed under an Act of the General Assembly, approved April 22, 1901, do hereby certify that we have this day examined and verified the Diploma of WE Williamson, M. D., age 42 Finley Tenn of Dyer County, and find the same to be genuine, and issued by the Memphis Hosp Med College a Medical College located at Memphis, which College is in good standing. Said Diploma is dated April 28, 1905 Said W. E. Williamson, M. D., having creditably passed the examination required by law before this Board, is therefore entitled to all of the privileges of the Act above referred to, when this Certificate of Permanent License is properly recorded in the County of the State of Tennessee in which he proposes to practice.

This Certificate of License is void if not recorded within three (3) months from its date of issuance.

Given under our hands and the seal of the State Board of Medical Examiners of Tennessee, at Nashville, Tennessee, this 27 day of April, 1905.

NOTE 2: This certificate was found in a book called “Register of Physicians” in the basement city building, which is owned by the Court of Dyer County. They would not allow me to take to book or transcribe it, but were kind enough to allow me to make of copy of the certificate.
