I hereby certify, that I attended deceased from ___ 191_, to Feb 6, 1916, that I last saw her alive on Feb 6, 1916, and that death occurred, on the date stated above, at __m. The cause of death was as follows: Old age (signed) Chas A Turner, M.D. Feb 23, 1916 (address) Dyersburg, Tenn 18. Length of Residence (for hospitals, institutions, transients, or recent residents): 19. Place of Burial or Removal: Mt. Hope Date of Burial: Feb 23, 1916 20. Undertaker: H. R. Meriwether Address: Feb 23, 1916 This is a transcript of the death certificate I received for Susan Emma J. Reagan Gambell, however, I consider the information recorded to be questionable. The dates match with the information I have for Susan Gambell, but she is listed here as Mary, and her parents information doesn't match the maiden name I have for her or what is recorded on the death certificate of her son John Gambell. I'm guessing her information was mixed up with that of someone else who died around the same time as I don't see two 97 year, 9 month old women named Gamble dying in Dyer County on the same day. |
