Postmaster,
Union City, Tenn.

Dear Sir:

Mrs. Mary A. Jewell, a pensioner, has not sent in her voucher for December 1909.

We suppose she has died or removed from the State. In order that we may correct our roll, will you kindly make inquiry of a carrier, justice of the peace, or a school director of her district and let me know the result of the inquiry on the back of this sheet?

The last address given by her was Union City Tenn.

In case of death the legal representative is entitled to the pension for the full quarter in which the pensioner dies, upon making an affidavit before an officer using a seal, stating the date of death and name of the legal representative.

I enclose stamp for reply, and will appreciate your courtesy.

Yours truly,
Frank A. Moses
Special Examiner.

Dear Sir,

I am informed by W. W. Jewell son of Mrs. Mary A. Jewell, that his mother died Aug. 26th 1909, and that he returned her last check to you some time ago.

Very respectfully,
G. T. Taylor, P.M.
