Mrs. Lela Forrest Haynes of said State and County, desiring to avail herself of the Pension allowed to Indigent Widows of Soldiers, under Act of General Assembly, approved April, 1905, being Chapter 202 of the Acts of 1905; Chapter 103, Acts of 1909; and Chapter 7, Acts of 1911, hereby submits her proofs, and after begin duly sworn true answers to make to the following questions, deposes and answers as follows:
1. What is your full name and where do you reside? (Give State, County and Postoffice.)
ANSWER: Gadsden Tenn. RFD 1. Lela Forrest Haynes
2. How long and since when have you been a resident of this State?
ANSWER: 49 years of age and a resident of Tennessee all of her life.
3. When and where were you born and what was your maiden name?
ANSWER: Benton County Tennessee; Maiden name: Lela Forrest Cole.
4. When and where was your husband born -- state his full name, and when and where were you and he married, and who performed the marriage ceremony? (Attach certified copy marriage license in every case.)

5. When and where and in what Company and Regiment did you husband enlist or serve during the war between the States?

Will Yearwood, Undertaker of the said State and County, having been presented as a witness in support of the application of Mrs. Lela Forrest Haynes for a Pension under Chapter 202, Acts of 1905, Chapter 18, Acts of 1909; and Chapter 7, Acts of 1911, and after being duly sworn true answers to make to the following questions, deposes and answers as follows:
1. What is your name and where do you reside?
ANSWER: Will Yearwood, Alamo Tennessee
2. Are you acquainted with the applicant, Mrs. Lela Forrest Haynes
ANSWER: Yes
If so, how long have you known her?
ANSWER: Only a few weeks
3. Where does she reside, and how long and since when has she been a resident of this State?
ANSWER: Near Gadsden Tennessee. I don't know how long she has been a resident of the State.
4. When and where was she born?

5. Were you ever acquainted with her husband?

Haynes, ____ lots, $None, _____ acres, $None; real estate and personal $None. Neither was there any property assessed to W. A. Haynes

Crockett County
I was personally acquainted with W. A. Haynes of Crockett County Tennessee and can state on information that he died in this County June 13th 1930. He was very feeble the last time I saw him and shortly thereafter I heard he was dead. I am acquainted with his widow Mrs. Lela Forrest Haynes. They were very poor people and had no property or means. I assisted W. A. Haynes without compensation secure a pension and have assisted his widow, also without compensation prepare these papers.
